Instructions for filling out this patient form

  1. Answer honestly as many of the following questions as you're willing to.
  2. People can see your profile name, so skip anything too personal.
  3. You agree that we can, and will, eventually reveal these answers to other people. Lots of other people.
  4. We add your picture and profile into the patient pool. Randomly, it will be displayed in the game under the questions you publish the answers to. Also displayed will be someone who answered the question the other way, and takers will use your photos and perhaps profiles to divine who said what.
